Checks / Titles, headings and schema
Form field without a name
A field with no label, aria-label or title is announced as just an edit box by a screen reader and cannot be picked by voice control (WCAG 1.3.1 and 4.1.2, Level A).

What to change
Give the field a visible <label for="id"> or an aria-label. A file input that a button opens should instead get aria-hidden="true" and tabindex="-1".
